All appointments made with Hattie's Esthetics LLC require a card number on file to reserve an appointment time. For all full sets of lash services, a 50% deposit is required at the time of booking. The 50% remaining can be paid at your appointment or you have the option to pay in full when booking.

All cancellations or rescheduling should be made 24 hours before the appointment time. All cancellations, rescheduling, and no shows that happen less than 24 hours before the appointment time result in a 50% cancellation fee of the service booked. If you are late to an appointment by 20 minutes, it would result in a cancellation and fee of 50% of the service.

I agree to the terms above and give permission to charge my card on file for 50% of the service in the case where I no show, cancel, or reschedule less than 24 hours before my appointment time. I also agree to the cancellation and payment of 50% of the service if I am more than 20 minutes late to an appointment.

I have voluntarily elected to undergo this treatment/procedure after the nature and purpose of this treatment has been explained to me.

I understand and acknowledge that there are risks involved with the treatment I will be receiving. Although it is impossible to list every potential risk and complication, I have been informed of possible benefits, risks, and complications, and I have had the opportunity to ask questions regarding these risks and other possible complications.

I also recognize there are no guaranteed results and that independent results are dependent on age, skin condition, and lifestyle and that there is a possibility I may require further treatments of the treated areas to obtain the expected results at an additional cost.

I have read and understand the post-treatment home care instructions. I understand how important it is to follow all instructions given to me for post-treatment care. In the event that I may have additional questions or concerns regarding my treatment of suggested home product/post-treatment care, I will consult the esthetician immediately.

I have also, to the best of my knowledge, given an accurate account of my medical history, including all known allergies or prescription drugs or products I am currently ingesting or using topically.

I have read and fully understand this agreement and all information detailed above. I understand the procedure and accept the risks. I agree I will assume the risk and full responsibility for any and all injuries, losses, side effects, or damages which might occur to me while I am undergoing this procedure. I do not hold the esthetician, whose signature appears below, responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed today.

I understand that there are risks associated with having eyelashes applied to and/or removed from my natural lashes.

I understand that if I experience any pain or discomfort throughout my session, I will immediately inform my technician.

I understand that as part of the procedure, eye irritation, pain, itching discomfort and in rare cases, eye infection may occur.

I understand and agree that if I experience any of these issues with my lashes, I will contact my technician and have the eyelash extensions removed immediately and consult a physician at my own expense.

I understand that even though the technician may apply and remove the eyelash extensions properly, that adhesive material may become dislodged during or after the procedure, which may irritate my eyes or require further follow up care.

I understand and agree to follow the aftercare instructions provided by my technician. Failure to follow the aftercare instructions may cause the eyelash extensions to fall out.

I understand that in order to have the eyelash extensions applied to my lashes I will need to keep my eyes closed for the duration of 60-240 minutes during the procedure. I also understand that I will need to be lying in a reclined position. Any medical conditions that might be aggravated by lying still for a prolonged period of time may mean that I will not be able to have the procedure performed on my eyes.

This agreement will remain in effect for this procedure and all future procedures conducted by my technician. I understand that this agreement is binding and that I have read and fully understand all information above.

I release my technician from all liability associated with this procedure. There are no guarantees for the binding time length of the eyelash extensions.

I understand that it’s up to the technician to determine what service is appropriate. For example, the technician determines whether it’s a 3 week fill or full set depending on how many extensions are left when they come in.

I understand that no sexual suggestive remarks or advances made by me will result in immediate termination of any session.

I consent to having before and after pictures and videos taken for advertising and marketing purposes and that these pictures & videos may be posted on social media.
